For Local Employees:
Photocopy of 2 valid IDs with 3 specimen signature
*Company ID
(and any of the following:)
*SSS/GSIS/Voters ID/Postal ID/Driver's License/TIN ID/UMID/etc.
Latest Proof of Billing: (any of the following:)
* Meralco Bill / Water Bill / Cable / Telephone Bill
Additional Requirements:
*Barangay Clearance (xerox/original)
*Latest Certificate of Employment with Compensation (COEC) -
original notarized
*Latest 1 whole month payslip (Original)
*Latest Income Tax Return (ITR) - xerox copy if applicable
*Photocopy of Marriage Contract (if Married)
*Photocopy of valid IDs of Spouse with 3 specimen signature
*Photocopy of Birth Certificate (if Single)
*6 pcs. 1x1 colored picture
*Latest Community Tax Certificate (original/xerox)
For Overseas Filipino Workers (OFW):
Photocopy of 2 valid IDs with 3 specimen signature
*Passport ID (with stamp of Arrival/Departure)
(and any of the following:)
*SSS/GSIS/Voters ID/Postal ID/Driver's License/TIN ID/UMID/etc.
Latest Proof of Billing: (any of the following:)
* Meralco Bill / Water Bill / Cable / Telephone Bill
Additional Requirements:
*Barangay Clearance (xerox/original)
*Notarized Special Power of Attorney (SPA/SPA with Red Ribbon)
*Photocopy of any 2 valid IDs of the appointed SPA
*Latest Employment Contract (Photocopy)
*Photocopy of Marriage Contract (if Married)
*Photocopy of valid IDs of Spouse with 3 specimen signature
*Photocopy of Birth Certificate (if Single)
*6 pcs. 1x1 colored picture
*Latest Community Tax Certificate (original/xerox)
For Self-Employed:

(any of the following:)
